  • Bilateral Post-Operative Rehabilitation/Orthotics – 60 minutes

Patient Forms

New Patient Form

Please arrive five minutes prior to your appointment to allow enough time to fill out our new patient form, along with any other relevant information or documents needed for your appointment.


Department of Veterans' Affairs (DVA)

Veterans Affair’s patients will receive appropriate forms from DVA. Documentation including a DVA number is required information without which, an appointment rescheduling or cancellation may occur.

Workers Compensation (WC)

All Workers Compensation/MVA Patients are required to send either an Approval Form or a 3A form from their lawyer/Insurer, accepting liability for the treatment provided.

 

Appropriate forms include a ‘Claim Number’, and a ‘Case Manager’. Without providing this information to the clinic prior to your appointment, will incur a rescheduling or cancellation of your appointment. If you do not provide the clinic with your claim number, you will be required to pay as a Private Patient until we receive the Approval Letter or 3A form.

Enhanced Primary Care/Chronic Disease Management Plan

Your GP may provide you with an EPC/CDM Plan prior to your appointment. These management plans issue Medicare rebate for up to five podiatry appointments per year. Patients will pay the full cost of their appointment and receive rebate through Medicare within the following two days. Please bring a copy of your care plan to your appointment and we will be able to process your claim through Medicare.


As of July 2023 the current Medicare rebate allocated for EPC/CDM patients is $58.00.

Fees & Payments

All patients are required to pay on the day of their appointment. Private Health rebates are accepted, via our HICAPS machine.

Payment options include: Eftpos, credit card and cash. Unfortunately, we do not accept Cheques.

Booking Policy & Confirmation

Please arrive a little before your scheduled appointment time as there can be some documentation or other matters to attend to before your appointment. It is also helpful if you bring all the necessary documents to ensure a smooth process.


Mobile phone SMS or text messages are automatically sent from our computer the day before your appointment. Please note that if you do not respond to the message with confirmation, we may contact you to check that you are still attending your appointment.


Patients without mobile phones need to phone the rooms to confirm one working day prior to the appointment. Alternately you can email a confirmation to the practice.

Late Arrival Policy

It would be greatly appreciated if you could allow ample time for parking and filling out new patient forms before your scheduled appointment time.


We do understand that circumstances may arise unexpectedly, however, late arrivals tend to compromise the punctuality of service we are able to provide to you, as well as our other patients. 


If you arrive late we reserve the right to reschedule or reduce the length of the consultation or procedure to ensure that their next scheduled appointment does not have to be put behind.


If you believe you will be late it would be appreciated if you could call ahead and advise the reception as alternative arrangements may be possible.

Cancellation Policy

The Foot & Ankle Rehabilitation Clinic requires a minimum of 24-hour notice if you wish to cancel your appointment. This provides the practice with an opportunity to offer the appointment to other patients from our waiting list.

 

If you cancel or do not attend a scheduled Podiatry appointment without providing 24 hours’ notice, a fee of $30 will be charged. It is in your interest (as well as ours) to give as much notice as possible of any appointment changes.

 

For Workers’ Compensation or Motor Vehicle Accident patients, please note that insurers do not cover charges for non-attendance, and a charge of $90.00 will be payable for a missed appointment.

What to Expect

During your initial consultation at Foot & Ankle Rehabilitation Clinic we will ask the following:


  • Request detailed information about your symptoms
  • Take a medical history of your current and past ailments
  • Perform a thorough physical and/or other examinations

What to Bring

  • Referral Letter (GP or Physio).
  • Medical Information.
  • X-Rays, MRI or CT Scans.
  • Current Footwear and Orthotics (even if in a Post-Op Shoe/moon boot).

Personal Information

  • Private Health Insurance Card.
  • DVA Card.
  • Pension Card.
